CST Coordinator/Case Aide

Under supervision of the CFU Supervisor, the CST Coordinator and Social Work Case Aide position will provide service to clients in the Children and Families Unit.  Position provides case management to assigned clients in need of services to youth, families and adults populations.  Considerable independent judgment and discretion are required in working with individuals, families, providers, and community agencies.  Approx. 70 % of the workers time will be spent on the coordination of services for the CST initiative through direct contact with families who are enrolled in the initiative, facilitating teams, developing a plan of care for each family and fostering cooperation between individuals, agencies and systems to meet the needs of the families served.  Approx. 30 % of the workers time will be designated as a case aide to assist social workers in non-technical services such as transporting clients, conducting supervised parent-child visits, face to face contacts  and making reports to the social worker, providing assistance for daily living skills and parenting i.e.: transportation for shopping, Dr. Appointments, school meetings, etc. and performing routine clerical and related tasks, such as WiSACWIS documentation, mandated notification letters and other clerical duties related to case management. This position will further assist social workers in voluntary offer of services by contacting families following agency referrals.

Bachelor’s degree in social work or social service related field.  Additional training, education and work experience that provide required knowledge and abilities related to working with children and families with multiple behavioral needs.    One to two years of general work experience, preferably in a human or social services setting.
